In checking out my facts in several locations it seemed to be that you have a couple of choices of initial order: first, last, middle with the last name initial slightly larger; or first, middle, last with the initials all being the same size.
The only exception would be for a married women who uses her maiden name as well as her married name, then the order is first, married, maiden. You will see that pretty much anything goes ~~ if it is creative enough.
